Description:

The web provides an excellent means of gathering feedback and responses from readers at very little cost. However, creating the instruments to gather the feedback typically falls on the shoulders of IT staff. This can create a barrier to the creation and use of such instruments, regardless of their utility.

fbx_Formbuilder allows registered end users to create their own forms, publish them, and receive the responses, without requiring a knowledge of HTML or server technology. Through a form-based user interface, users create new forms; add, edit, delete, and reorder questions; publish the form to the web server; and view or download the results. They may also choose to have the results emailed to them as they come in.

fbx_Formbuilder can be used to create surveys, feedback and comments forms, event registration forms, and more. fbx_Formbuilder supports several question types, including single-line free text, multiple-line free text, select only one from a list, select one or many from a list, matrix type (typically applying a scale to several consecutive items), and comments or instructions. The form creator can also require that individual questions be answered prior to form submission.

This project uses Fusebox 5 in a model-view-controller structured application. It is written procedurally (i.e., it doesn't use CFCs for the model)
